# Form Designer

Go to Repository.

Click on “Add Repository”.

There are four type of template to create form.

* Generic Repository
* Helpdesk Ticket
* Survey
* Field Ticket

## **Generic Repository**

Enter Title, description and select template as “Generic Repository”.

You can add name of the section.
